Pursuant to art. 13 and 14 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation - GDPR), I inform that:

• The Administrator of Personal Data is Jack Rost Jacek Rostkowski, ul. Młynarska 17B, 69-200 Sulęcin, Poland
• The personal data will be processed in order to fulfill the contract/for marketing purposes
• The recipients of personal data are: Jack Rost Jacek Rostkowski and companies cooperating in the execution of the agreement
• From the moment of their transmission personal data will be processed within a maximum period of five years starting from the beginning of the year following the year in which the data were made available
• You have the right to access, rectify, erase or restrict your data, as well as the right to object to the processing and the right to transfer your data
• You have the right to withdraw your consent to the processing of personal data at any time without affecting the lawfulness of the processing carried out on the basis of your consent prior to the withdrawal
• You have the right to lodge a complaint with a supervisory authority
• The provision of personal data is a statutory/contractual requirement. You are/are not obliged to provide personal data. The consequence of not providing the data is a potential lack of possibility to use the service offered by the personal data administrator
